Thank you for booking.
Please read the following information.
Please complete your medical details
Please click on this link to open a short questionnaire which will provide some basic medical details. These are securely stored, and it only needs to be done the first time you book with us.
Directions
For clinic appointments you will have a confirmation email with directions (it may be in your junk mail if you don’t see it). Please head to Harrow House in the Haslemere Waitrose car park (GU27 2AB), you should see the Lisle Medical sign on the wall above the gate. You can park for free in our gated car park, although it can be tight sometimes so please be careful. If you have a large car you may want to consider using the main car park. There is step-free access to the clinic.
For home visits we will use your postcode on satnav - please let us know if we need to do something different, or about any access arrangements.
Additional requirements from us
If you require anything from us that we may not have already asked about, for example any communication assistance (hearing aid loops, interpreter services), or you have mobility or care needs that we may need to know about before your appointment, then please let us know so that we can optimise the appointment and get as much from it as we both would wish.
Appointment timings
We try our best to keep to time, but please allow 15 additional minutes for us to arrive for a home visit due to unexpected travel delays. In clinic if you have been waiting more than 5 minutes past your appointment time please knock on the clinic door.
We allow 30 minutes for a consultation. If we need to run over then we will if the diary permits, or we can pick up the conversation later. Charging policy is based around common sense and an acceptance that not all problems are easy to define in a limited time period, so please don't feel rushed.
Cancellation
Of course if you no longer need an appointment please let us know. We do ask that you give as much notice as possible. If already en route for a visit we may apply a limited charge for time and expenses.
Chaperones
We usually can't offer you a formal medical chaperone, and so some examinations are not appropriate. Even so, please feel free to invite a friend or family member along if you would feel more comfortable. If you have any concerns you must discuss them with the doctor or send a message prior to the appointment.
Feedback
Should you wish to provide feedback after your appointment we always welcome this, particularly in writing. If you have concerns or a potential complaint we would always prefer that you raise this as early as possible, even at the time. Please refer to our complaints policy at this link.
Identification
Please have something ready to confirm your identity if this is your first appointment with Lisle Medical. Driving licence, passport, formal ID card, bank card, utility bill - any of these will suffice.
Insurance
Most UK insurance policies do not include general practice consultations. If yours does then you should be able to pay and claim the cost back - we can provide an invoice and fill out any forms required.
NHS medical records
We don't need these, but if you think it would help you can access some records through the NHS app. An NHS practice will need to authorise sharing of your data with the app which can take a couple of days. If you need us to share your records with an NHS GP please let us know - we have access to NHS email and will usually be able to get it to them quickly.
Payment
Usually we email an invoice after the appointment which enables you to pay online, but we can accept cash, cheque and all major cards either in person or by phone if that’s easier for you. For some appointments we may request payment up front.
Prescriptions
We can issue prescriptions in various ways - printed, emailed to a pharmacy or via some efficient third party services. We regret that we cannot issue controlled drugs and certain other medications to patients without a face-to-face appointment and sight of identification.
Safety
Staff safety is paramount and is tracked at all times (location only via GPS, no device has any listening or recording capability). Please be aware that abuse of staff will not be tolerated by Lisle Medical.
